Forms of Payment

I accept debit and most major credit cards, as well as HSA and FSA cards. 

The only insurance that I am able to accept at this time is the Oregon Health Plan (OHP), Open Card. Please inquire if you have questions regarding any CCO's (such as IHN), through which you receive your OHP services.
